There is a self-contained, unique application form for each category. (Note: the “Recording” application covers both the Recording and Major Recording categories.)

HOW TO SUBMIT AN APPLICATION
1. Select the category in which you are competing. In doing this, refer to the category definitions presented in the MAC AWARDS RULES. If you’re not sure which category is right for you, go with the one that strikes you as most appropriate and feel free to use the Comments field to explain your selection or express your concerns.
2. Click on the category from the links below to download the respective application (PDF file) to your computer.
3. Open the downloaded file. Using your computer, enter the requested information.
- The application form will automatically adjust font size and number of lines as needed to fit each field.
- You can pause entering information by saving and closing the partially completed PDF file. Pick up again whenever you’re ready.
- Each application form contains information about the supporting documentation required for the chosen category and detailed submission instructions.
If you are submitting in more than one category, each category requires a separate email, with the completed category-specific application form and supporting documentation attached.
